Intimate and Personal Care Agreement – School Age Child

This Intimate and Personal Care Agreement is for School Age Child Services who have a child that may require help with eating, drinking, washing, etc. Alternatively, the child may require supervision with personal care such as changing from soiled clothes to clean clothes. This agreement includes important definitions of intimate care and personal care. It also outlines the step-by-step procedure of intimate and personal care for the individual child. The agreement includes a statement of intent which aims to safeguard the rights and promote the welfare of children and young people. The service provider is responsible to ensure the agreement is communicated to and signed by the parents/guardians. The service must also ensure the relevant staff are knowledgeable about the childs personal care and that the child’s individual concerns are considered.
