Medical and Health Forms

Medical and Health Forms are an important at any Early Years Service. Do you have children attending your service with a medical condition such as epilepsy, diabetes, a nut allergy or asthma? Or do you have a child with a more serious condition such as Long QT Syndrome or Cystic Fibrosis?
If so you should have an emergency care plan in place and this should be authorised by the child’s parents/guardians and medical practitioner.

Your staff need to be confident that they know exactly what to do in the event of an emergency. We have developed a standard letter to give to parents along with a medical questionnaire and an emergency care plan for the most common conditions.

It is important to be guided by the parents and collate all the relevant information so the child can be cared for appropriately and to also review the information regularly to ensure that nothing has changed. The care plans should be signed by parents and where appropriate the child’s Doctor.
