10 Record Keeping Mistakes in an Early Years Setting!

Not signing or dating records
Using pencil, not pen to complete records

Missing information on the child record form such as the dates of immunisations or permissions
Not including start and finish dates on child registration records
Not having employment references dated and signed with full name and address of referee
Not using full names in accident and incident forms when listing who was present or who witnessed the accident
Not recording the “outcome” of the administration of medication, did the child’s temperature reduce, for example?
Not updating records regularly. Children acquire conditions, allergies and illnesses. Records should be up-to-date, accurate and shared with those who need to know on a regular basis
Not ensuring parents sign records where appropriate (e.g. accident records, medication records)
Displaying private data in a public forum, without authorisation
