Managers should aim to carry out a Performance Management review more than once annually. Try implementing the 4X4 model tool using this 2-page template as a way of improving performance with staff members. It will help with increasing engagement as well as encouraging a Growth Mindset among your employees. The aim is to carry out these 4 simple but effective Questions, 4 times per year. Two of the questions look back (Reflection) and the other two questions look forward (Future Expectations).
The form is a Word document that allows you to edit to suit your own service needs. You can add your service logo, staff name, position and date of review. It then has boxes for the 2 questions in relation to Reflection (Looking Back). It gives plenty of space for the staff member to fill in their answers. Next it has 2 questions in relation to Future Expectations (Looking forward), again with plenty of space for staff members to fill in their answers.
