Early Years services are required to gather COVID-related information for all the children returning to their service after the Christmas break. The Post-Christmas COVID-19 Response Form provides you with an online form you can share with families of children in your service. Parents or guardians can then fill in the relevant details and the responses will be automatically emailed to your designated contact email.

We also provide online access to all the responses together (in case any of the responses “go missing” by getting spam filtered or any issue with email) for your reference for a brief period following usage of the response form.
